Change of shift chaos organization I was having issues getting reorganized during the last 4 hrs of my 12 hr shifts, much of it due to the ‘change of shift’ chaos happening on my new unit. SBAR report sheet As a new grad nurse, the SBAR format is the best way to communicate all the important items about each patient. This report sheet is thorough and helps me to pick out the things that I really need to pass on to the next nurse. This report sheet is great!
